Libra, my open-source PHP web file manager (versions 0.217 through 1.18, 2003-2008), later continued as osFileManager 2.x with a Google Code repo; it was distributed on SourceForge and osfilemanager.com.

Libra, Version After Version
Libra shipped as zip after zip. 0.217. 1.0. Then Libra PHP File Manager 1.0, 1.03, 1.05, 1.08, 1.17, 1.18. Then it became osFileManager 2.0, 2.1, 2.2, with a Static edition alongside it, and later moved from SourceForge to Google Code. July 2003 through October 2008.
Back then shipping meant building the archive by hand, uploading it and updating the version number on a page. A lot of those versions were me getting a bug report and putting out a new zip that weekend.
It was a PHP script that let a browser manage a server's files, and it ended up on a lot of machines. I learned most of what I know about permissions and untrusted input from people reporting problems with Libra.
